DICOM, which stands for Digital Imaging and Communications in Medicine, is a standard that was developed to “promote communication of digital image information, regardless of device manufacturer” within a radiological environment. It defines:
• How the radiological objects (Images/Reports) should be structured
• How radiological objects should be exchanged

The standard is both a file format and a transfer protocol, and it is not tied to a particular manufacturer, hardware device, operating system or software application. This is similar to other common formats such as JPEG image files, which are created and viewed by many applications with few interoperability problems. DICOM images can be displayed using a wide variety of systems, provided that the system includes the appropriate software (such as PACS), and the suitable equipment designed to meet the needs of the physician, the facility and the modalities.

Display Systems include such things as monitors with their associated driving electronics, and printers producing films which are placed on light-boxes or alternators. Soft copy reading is the preferred method of viewing digital images. This minimizes the storage space required for hard copy films, as well as reducing the cost of consumables, while improving workflow and productivity. In medical imaging, it is important that there be a visual consistency in how a given digital image appears. A digital image which has good diagnostic value when viewed on one device could look very different and have greatly reduced diagnostic value when viewed on another device.

There are two classifications of digital viewing workstations, Diagnostic and Clinical (also known as Review). The distinguishing characteristics are resolution and functionality, based primarily on the monitor and the software. These workstations may be part of a PACS or may be third-party systems that work in conjunction with a PACS. The workstations are capable of receiving DICOM images from a modality or PACS and producing additional radiological reports, measurements and images, which can then be sent to another DICOM device, such as an archive.
Commercial off-the-shelf displays have some of the characteristics that are appropriate for review and diagnosis of medical images, but they lack others, which should be of great concern to the physician providing the interpretation. Standard computer displays offer limited resolution with a form-fit factor (landscape) that is not optimized for diagnostic imaging. Medical grade displays, on the other hand, offer resolutions up to 2048 x 2560 (5 megapixel) in portrait or landscape that corresponds better with the image format of the medical images. Higher resolution allows the physician to see much more detail without panning or zooming the image, enabling him/her to make clinical, even life critical decisions more quickly.
